Follow these steps to gain access to your patient portal today.
- Please ensure we have your current email address on file so we can send you an invitation to enroll. Contact FollowMyHealth patient support at
(888) 670-9775 should you have questions. - Once you receive the email from FollowMyHealth ([email protected]), click the registration link to begin the enrollment process.
- Follow the prompts to create your account using a unique username and password.
- When prompted, enter your invitation code (your four-digit birth year in the format YYYY). Continue following the on-screen instructions to complete your registration.
- Download the FollowMyHealth app from the Apple App Store or Google Play Store by searching for “FollowMyHealth.” Log in using the username and password you created during registration.
- You can securely access your patient portal anytime from your mobile device or computer.
Already have a FollowMyHealth account?
- Click the link in your invitation email and select, “Sign in and add this connection.”
